Dear Sir or Madam,

You are invited to participate in a study related to counseling and psychotherapy. I hope to learn how mindfulness, empathy and cognition relate to one another in counselors, therapists or trainees. You were selected as a potential participant because this listserv/website is open to research participation and may include practicing counselors, therapists or trainees.

If you decide to participate, please complete the enclosed survey (hyper link is below). Your return of this survey is implied consent. The survey is designed to collect basic demographic information (no identifying information will be requested) and measure aspects of mindfulness, cognitive empathy, emotional empathy, and cognitive reappraisal in counselors or therapists who are currently working with clients or patients. It will take about 15 minutes to complete the survey. No benefits accrue to you for answering the survey, but your responses will increase our understanding of mindfulness, empathy, and cognition. Any discomfort or inconvenience to you derives only from the amount of time taken to complete the survey.

Any information that is obtained in connection with this study and that can be identified with you will remain confidential and will not be disclosed. However, this study will not collect data that includes identifying information or numbers. All data will be kept in a secure file and laptop, or locked cabinet.

Your decision whether or not to participate will not prejudice your future relationships with Minnesota State University, Moorhead. If you decide to participate, you are free to discontinue participation at any time and without prejudice.

Please feel free to ask questions regarding this study. You may contact Dr. Duane Halbur, the Chair of MSUM Counseling and Student Affairs Department, at halburdu@mnstate.edu or 218-277-2127. Any questions about your rights may be directed to Dr. Richard K. Adler, Chair of the MSUM Institutional Review Board at 218-477-2474. Thanks for your time!
